Your care Patient, Children's and Young Persons Specialist Bereavement Support Our team offer specialist bereavement support, which is an opportunity to explore feelings, thoughts and memories. Please note that the team are only able to accept counselling referrals for those who are already known to the Hospice. Our specialist team offer bereavement support to adults living with a life limiting illness, from diagnosis through to end of life or into survivorship. Bereavement support is also extended to those who are already known to the Hospice and may have experienced a bereavement within the last 3 years due to a life limiting illness. Bereavement support is also provided to children and young people up to 19 years of age, who are experiencing a bereavement and have a family member who is known to the Hospice. We use your initial assessment to review the level of support that we can offer, to ensure the most beneficial support is given. Referrals Our teams are on hand to answer any queries you may have and assist you with every step of your referral. Please note that the team are only able to accept counselling referrals for those who are already known to the Hospice. Referrals must be made with the permission of the client/parent/main carer and parental consent is required for clients under the age of 16 years.
